Accompanying family members
Spouses and minor children of persons in the cultural and artistic (D-1) through specific activity (E-7) statuses who do not have a spouse (except for persons in the technical training (D-3) status).
Companions of specialized personnel
Spouses and minor children of foreign nationals in the teaching (E-1), research (E-3), technical guidance (E-4), specialty occupation (E-5), and specific activity (E-7) categories who have been issued a GOLD CARD for employment in the field of high science and technology.
Required Documents
Application for authorization to issue a visa, a copy of your passport, and one standard-sized photograph.
